So we have successfully been able to add accounts with API and enable Intune on. We are doing it Rewst and doing it based on permissions with have with GDAP. Got the accounts setup, Intune working and enabled but Defender doesn't enable, we have to click the button. We are already assigning all the permissions the API needs to authorize things but still not working. Anyone doing this already or able to advise on how to help?

If I were trying to do this, I would work around the issue by using something like PowerAutomate Desktop to take a list of the Account IDs and cycle through each account with the URL (https://nmmurl.mycompany.com/accounts/##/settings/integrations) and enable the feature. Although, depending on the number of accounts you're needing to do this for, "the juice may not be worth the squeeze" to automate it if you're needing to have someone learn how to use PowerAutomate.
You could do a slightly less automated way and open up all of the integration screens in a separate tab and have someone click through and manually enable Defender for each account.
All of that being said, I don't know of an "official" workaround for this (maybe another partner does?), right now, but I did hear from the product team that they have heard about several API limitations like this and are working improve the API functionality in future versions.
It could be as early as the 5.6 release, but it could also be later.
I would keep an eye on the Release Notes to see when the functionality is added.
